refactor(topology)!: rename node info revision

Replace the epoch/revision freshness tuple with nodeInfoRevision.

BREAKING CHANGE: topology API clients must use nodeInfoRevision;
nodeInfoFullRevision and nodeInfoFullRevisionEpoch are removed.
